Home Health Care Assessment

We start the interview process by conducting an in-depth assessment of you or your loved one’s lifestyle, medical conditions and the level of assistance needed to help them remain independent at home.  We will meet with you and your family and jointly determine the scope of care.

Our Promise

We understand that determining the type of assistance your loved one needs and who can best help may be a daunting task.  Our commitment to you is that if we cannot provide the service you require, we will tell you upfront, and refer you to someone to who may be able to better serve your needs.

During the meeting, we fully explain the services available, answer any additional questions you have of us, and help with the important elderly care decisions faced by the family with a loved one in need.  The issues covered during an assessment include:

• Matching the appropriate certified caregiver based on needs, personality and location
• Discussion of overall health and medication issues and SafeSenior Services
• Procedures for emergency response should that be necessary
• Discussion on any special dietary or nutritional needs
• Home cleaning issues
• Transportation needs
• Costs of services, including financial aid that may be available to your loved one
